Forms

The Forms feature is useful for gathering information from students, such as learning about student interests or collecting course feedback. Forms function in the same way as do Tests and offer most of the same options. The important distinction between the two is that Form questions are not assigned any point values and cannot be graded. 

You can decide whether or not the form should be anonymized. This helps ensure that Forms can be used when you need to know the student who responded (such as student information, group peer assessments, or clinical placement preferences) and when you want them to be anonymous (such as mid-semester course feedback)

Creating a Form

Forms are created in the same way as tests:

  1. On the Course Content page, click the plus (+) icon wherever you would like to place the Form
  2. In the side panel, click Form
  3. Type a name for the form and press enter
  4. Click the plus (+) icon to add a question, upload a file, or add a page break

Question Types

A Form can use any of the question types available to a Test and an additional Likert Question type. Questions do not have point values on Forms. Question types available include:

Viewing Results

You can view individual results by going to the Form and clicking the Submissions tab, or by going to the Gradable Items view of the Gradebook and clicking on the name of the Form. 

You can download all of the results as a spreadsheet or CSV by going to the Grades view in the Gradebook. Click the column header for the Form and select Download Results from the menu. You can use the default options in the side panel or customize based on your need (the defaults will download a spreadsheet with each student as a row and their responses as columns). Click the Download button.
