A template is not required. You can write your evaluation directly in the Collection Tool comment box, keep using a form you already know, or adopt a new one. This page helps you do any of the three. Whatever you choose, the portal records your selection so the self-evaluation and your evaluation use the same criteria.

Supportive Professional Staff (SPS)
Continue to use a narrative-based evaluation. Enter it directly in the ePerformance Collection Tool comment box, or upload it as a Word or PDF document.
Current Civil Service
Continue to use your current form, move to a narrative model, and capture your evaluation in the Collection Tool, or build a new form (or replicate your old one) using the Kuali form builder. The decision is yours.

No Longer Sent, But Built

Evaluation forms are no longer sent to supervisors. You select an existing template, build your own, or, for SPS employees, use a narrative. The portal records your selection.

Changing competencies? Competency changes apply to the upcoming evaluation year, not the year being reviewed. Discuss any changes with your employee — their feedback is encouraged, and the supervisor makes the final selection.

These templates are built using default performance factors for their respective classifications. You can customize the evaluation tool by including additional performance factors. Once complete, the generated file can be uploaded into the evaluation portal.

Use the standard SPS template or an approved division variant, based on the position description.

Required on every evaluation: Supporting Student Success is a University Goal and is built into every template automatically. It does not count toward your five-to-ten competency selection and cannot be removed. Narrative evaluations must also address it.

Every evaluation at NIU, regardless of role or template, includes how the employee supported student success in their own context. It is a University Goal established through shared governance, and it is built into every template automatically.

The template documents the ratings. The narrative explains them. You write an overall performance narrative; if the evaluation uses a template, attach the completed template and both become the record. For SPS employees using a narrative evaluation, the narrative and overall rating alone become the record. The requirements below apply to every evaluation, template or narrative.

  • Address every duty area and competency on the selected template.
  • Speak to how the employee supported student success in their own context.
  • Support each judgment with a specific example from the year.
  • Describe behavior and results, not personality or intent.
  • Contain nothing the employee is hearing for the first time.
