Appeals

This appeal form is only to be used by individuals appealing the results of a decision made pursuant to the university’s Sexual Misconduct Policy and Procedures or Nondiscrimination, Harassment and Retaliation Policy and Procedures.

Only the complainant or respondent may file an appeal. Appeals must be submitted within the time-frame indicated within the relevant policy.

This form is not to be used for a grievance/complaint process provided for elsewhere by the university including, but not limited to, the faculty/staff university grievance, grade appeals, collective bargaining/union grievances and student grievances.
