Student Conduct History Request Form

Student Conduct maintains the academic and non-academic record of students referred to our office who are in some way involved in a potential violation of the NIU Student Code of Conduct and/or other applicable university policy. Student conduct information is not considered directory information under the Family Educational Rights and Privacy Act of 1974, and therefore is considered protected.

Student Conduct recommends that university departments obtain the permission of the student to release non-directory information. Northern Illinois University departments may seek the written permission of the student to check conduct history. University departments must have a legitimate reason for checking the conduct history of a student, not simply because the department is curious about the student.

NIU departments may have the release of information on the departmental document which is retained by the department, or the department may have the student complete the NIU General Release of Non-Directory Information form.

General Student History Check Information

  • Departments may use this form to submit the names of multiple students (no limit of students per form)
  • Please allow up to two (2) business days for the student conduct office to check the conduct history of the students
