Report an Incident
General
Use this form to report general incidents of misconduct. Incidents include, but are not limited to:
- Alcohol use
- Drug use
- Housing policy violations
- Physical assault
- Organizational violations
- Hazing incidents
- Disruptive behavior
Sexual Misconduct (Title IX)
Use this form to report incidents of sexual misconduct and behavior that violate Title IX.
- Dating and domestic violence
- Gender-based discrimination
- Sexual harassment
- Sexual assault
- Stalking
Student of Concern
Use this form to report non-urgent and concerning observed student behavior. It’s sent to the Division of Student Affairs. For more information about when is the appropriate time to use this form, please visit: the Community of Care website.
Bias
Use this form to report a non-urgent incident involving bias. It's sent to the Office of Academic Diversity, Equity and Inclusion.
Freedom of Expression Event
Use this form to register a freedom of expression event that will occur in outdoor spaces on campus, such as sidewalks and common areas. It's submitted through Huskie Link.
Academic Misconduct
Use this form to report incidents of academic misconduct. Academic misconduct includes, but is not limited to:
- Cheating
- Plagiarism
- Use of unauthorized sources
- Prohibited behaviors noted on a class syllabus
Student Conduct History Check
Use this form when your department wishes to obtain conduct records of students.
General

Medical
Housing and Residence Life staff use this form to report individuals involved in medical or traumatic situations. It should be used anytime someone seeks or needs medical attention, regardless of transport to the hospital
Fire Alarm
Housing and Residence Life staff use this form to report fire alarm incidents in residence halls, including fire alarm activations and trouble alarms. This form should also be used when there any evidence of fire or potential arson, regardless of whether or not the fire alarm activated.
Bias
Use this form to report a non-urgent incident involving bias. It's affiliated with the Office of Academic Diversity, Equity and Inclusion.
Routine Maintenance Issue
Housing and Residence Life staff use this form to report routine maintenance issues in the residence halls. This form should not be used to report damage, destruction or vandalism to property. Issues of damage, destruction or vandalism should be reported through the general incident report, with the nature of the report set to Damage, Destruction or Vandalism.
Follow-up Incident Report
Housing and Residence Life staff use this provide follow up information for a previously documented incident.
