Student COVID-19 Vaccine Exemption

If you have a religious or medical reason for not receiving the COVID-19 vaccine, you need to submit that information through MyNIU and watch the status of your request to ensure that additional information isn’t needed, or your exemption request may be denied.

How to submit your medical or religious exemption through MyNIU:

  1. Log in to the MyNIU Student System.
  2. Choose “My Forms.”
  3. Choose “Fill out a new form.”
  4. Choose “Covid_19_Decline.”
  5. The Exemption/Declination Form will ask you to choose your reason for requesting an exemption. Each reason has additional information you will need to provide or be aware of.
    • If you select the medical exemption, you will need to submit a signed statement of medical exemption from your primary care provider.

    • If you select the religious exemption, you will need to submit this religious exemption form describing the grounds for religious exemption. Your objection must set forth the specific religious belief that conflicts with the immunization and may be personal and need not be directed by the tenets of an established religious organization. General philosophical or moral reluctance to allow immunizations will not provide enough basis for an exception to the requirements.

  6. Choose “Save” and then upload the image or scan of your statement if you are submitting a medical or religious exemption.
  7.  Choose “Save,” then "Submit." 
  8. You will receive a confirmation email that the form has been submitted.
