Plan Information

The NIU Student Health Insurance Plan is a PPO plan underwritten by Blue Cross Blue Shield and covers a student 24 hours a day, anywhere in the world.

ENROLLMENT IN THE PLAN IS AUTOMATIC FOR STUDENTS REGISTERED FOR 9 OR MORE ON-CAMPUS HOURS AND ALL INTERNATIONAL STUDENTS.  Please see Eligibility & Enrollment for more information.

Some of our coverages for the 2013-2014 school year:

• $500,000 annual maximum
• Most Preventive services (100%), most well-care (100%), most birth control (100%), most immunizations (100%) and allergy shots (80%) now covered
• Physical therapy & chiropractic care has unlimited visits at 80% coverage
• 6 mo pre-ex for students and dependents, 19 & over, waived with 6 months of consecutive coverage or creditable coverage
• ER copay is $150 per visit
• In network out of pocket maximum is $5000 per year
• Out of network deductible is $700 per year
• Out of network coinsurance is 60% of Usual and Customary charges
• Out of network out of pocket maximum is $8000 per year
