Provide the payroll office with a certified death certificate, a taxpayer identification number (TIN) for estate, and a circuit court order declaring heirship (a small estate affidavit or a copy of the last will and testament are also acceptable).
Have the beneficiary or TIN complete a W-9 form.
Return any NIU equipment to us or the department.
