The NIU Student Health Insurance Waiver Process
for Fall 2013 opens 7/8/13

• You will be able to access the waiver form only if you have been billed for the insurance.  If you have not been billed for the NIU student health insurance, then there is nothing to cancel. 
  
  
• Check your NIU email to see if you have been billed for the insurance
  
  
• To log into the waiver site, you will need your ZID and your birthdate (MMDDYYYY).  You will be able to change the password (not your NIU password), if you wish, after you enter the site.
  
  

After you submit the online waiver form:

• NIU will still verify the information you enter onto the waiver form.  
  
  
• If your waiver request is approved, you will receive a confirmation with three to five (3-5) business days.  The insurance fee will be removed from your bursar’s account after the confirmation is received.  Please keep a copy of the confirmation for your records. 
  
  
• If your waiver request is unable to be verified, you will receive a notice within three to five (3-5) business days, that we need additional or corrected information from you.   Please contact the student insurance office at 815-753-0122 for more information.
