Optional coverage for spouses and dependents under the age of 19 is also available. The student must be enrolled in the Plan and complete and return a dependent application (pdf) to the Student Insurance Office on or before the deadline (the fifteenth calendar day of the semester). Dependents must enroll each semester that coverage is wanted.
