SA Required Paperwork

 The Student Association requires completion of paperwork at the beginning of each semester to begin or continue as a recognized Sport Club student organization at NIU. The following forms and information can be found on the SA website:

Campus Recreation Required Paperwork

 Campus Recreation requires completion of paperwork at the beginning of each semester to begin or continue as a recognized Sport Club. New paperwork should be submitted when any changes occur. For example, when new members join the club, additional waiver, non-hazing, and roster forms should be resubmitted.
