This notice describes how medical information about you may be used and disclosed and how you can get access to this information.

Confidentiality

All health information is confidential.
If a patient 18 years or older wishes to release information to anyone outside of Health Services, including parents or an insurance carrier, he/she must sign an Authorization for Disclosure of Information. Parents or guardians are notified only in cases of emergency.

Parents and legal guardians of children under age 18 may access their student's information unless it is specifically protected by law.
