Appointment Times & Procedures

During the academic year, appointments are available at a variety of times Monday-Friday.  Appointment times are flexible.  Every effort will be made to accommodate reasonable day and time requests.  Requests can be made when dropping off the Client Assessment Questionnaire and 3-Day Food Log or by accessing the appointment request form online.


Procedures

Nutrition consultations are FREE for students!  Non-student members are charged a nominal fee. 

Students
Initial Private Coaching Session: Free
1st Follow-Up Session: Free
Additional Follow-Up Sessions: $10 each

Here's how to schedule an appointment:

  • Access the online appointment form for students or non-student members from the Procedures section above.
  • Complete the Client Assessment Questionnaire and 3-Day Food Log.
  • Submit the Client Assessment Questionnaire and the 3-Day Food Log via email (save as a PDF and attach to the email) at CampusRecNutrition@niu.edu.   -OR-  Drop the completed questionnaire and food log off at the Student Recreation Center Service Counter.  It is recommended that you put your information in a sealed envelope, or fold the forms together and tape or staple them shut. 
  • If forms are sent in the mail, they should be directed to Recreation Services, attn:  Assistant Director for Fitness/Wellness
  • Be sure to include your name and contact information on the inside of the forms. 
  • A nutrition staff member will contact you within two business days of receiving the forms via phone or email to schedule your appointment.
  • Sessions last approximately one hour each.  Follow-up sessions can be scheduled at the time of your initial appointment.
  • Please Note:  To guarantee that the nutrition staff has adequate time to analyze and assess information, food logs should be returned at least 72 hours prior to scheduled appointments.  Logs dropped off the day before or day of an appointment will not necessarily be reviewed in time to hold a scheduled appointment and may cause appointments to be re-scheduled.
