Accreditation Document Review and Approval
Use this form to submit accreditation documents to the Office of the Provost for review and approval. The accreditation documents submitted should be the final, edited version. All documents should be approved by the Office of the Dean prior to submission. Please allow 10 working days for review in the Office of the Provost.
