Patient Rights and Responsibilities

As a patient, you have
the right to:

  1. Be treated with dignity and respect in a non-discriminatory manner.
  2. Know the names and professional titles of the people serving you.
  3. Select and/or change your healthcare provider or request a second opinion.
  4. Privacy during your visit.
  5. Confidentiality of your personal health information.
  6. Participate with your provider in making informed decisions regarding your health care.
  7. Have all questions answered regarding your health care.
  8. Consent to or refuse any care or treatment.
  9. Review your medical record with a health care provider.
  10. Voice concerns or complaints about services or policies.
  11. To received written discharge instructions when you leave Health Services

As a patient, you have
the responsibility to:

  1. Provide complete and accurate information to the best of your ability about your health status.
  2. Follow the prescribed treatment plan, and ask questions if you do not understand.
  3. Report any significant changes in symptoms or failure to improve.
  4. Show courtesy and respect to healthcare personnel and other patients.
  5. Arrive for your appointment on time or cancel your appointment in advance.
  6. Accept personal financial responsibility for any charges not covered by the health fee.
