On August 5, 2016, the Illinois Department of Public Health changed the college immunization code (77 Ill. Adm.Code 694), to require:
Fall term enrollment: August 1st
Spring term enrollment: January 1st
Summer term enrollment: June 1st
Failure to provide the required documentation by the tenth day of the semester will result in a late processing fee and registration hold. You will be notified at your NIU ZID email account if any documentation information is incomplete.
You can submit verified documents from your high school, university or college as well as your health care provider, or you can use the NIU Immunization History form. Please contact us if you would like a copy of the form that would be filled out by your physician. You would then submit it to us when completed.
Your records are not complete until they have been reviewed and verified by Health Services.
You will be notified via your NIU Z-ID email account of your immunization status upon receipt and review. Please note that a physical examination is not required for admission.
Immunizations are available at the Health Services Preventive Medicine clinic on a walk-in basis. NIU Student Health Insurance does cover the cost of immunizations. Health Services does not provide 3rd party billing for non-Student Health Insurance plans. Please call 815-753-9585 for hours and current fee information.
Contact the Immunization Office 815-753-9585 or Health Services 815-753-1311 for exemption information, e.g. students born before 1/1/1957, Medical or Religious. Note re-entering students must contact Health Services to verify current immunization status.
Failure to comply with the above State of Il Immunization requirements will result in a late processing fee and registration hold.
