Permit Form for Internship, Independent Study and Thesis hours:  COMS 639, 697, 699 (PDF)

All M.A. students must file a   department committee approval form (PDF)  and a   course approval form (PDF) by the end of their first year of study. Both forms should be completed and signed by all members of your committee.

Thesis Students

These forms are required of all students who are completing a thesis as part of their degree program:

• Report of prospectus meeting (PDF)
• Thesis committee approval form
• Request to change a thesis committee
• Request to defend a thesis
• Results of an oral defense of thesis  (filled by your committee)

Non-thesis Students

All students who do not elect to write a thesis must complete a written examination.

• Competency area approval form (PDF)
• Report of Examination  (filled by your committee)
