Requesting Services
To request services, please call the clinic office at 815-753-0591 during our business hours. Our secretary will complete a form with you over the phone. She will ask for some demographic information and have you give a brief description of what type of problem(s) you are having. Sometimes an initial appointment can be scheduled during the phone call. Otherwise, you will receive a return call within a few working days. Our secretary will also provide directions to the Psychological Services Center and information about parking and related matters.
