Moderna COVID-19 vaccine clinics available for NIU employees and their immediate family members (ages 18 and older) April 20 and May 18

The DeKalb County Health Department is offering NIU employees and their immediate family members the opportunity to receive the two-dose Moderna COVID-19 vaccine. The first-dose clinic will be held from 9 a.m. to 2 p.m. Tuesday, April 20, and the second-dose clinic will be held from 9 a.m. to 2 p.m. Tuesday, May 18. Both clinics will be held at the Convocation Center.

When you
schedule your appointment for your first dose Tuesday, April 20, you are committing to return to the Convocation Center for your second dose Tuesday, May 18. You must receive the second dose of vaccine at the same location that you received the first dose because the allotments that the health department receives for the second dose are based on the number of doses administered at the first clinic.

Please take this important step to protect yourself, your fellow Huskies, and your family and friends. Remember, COVID-19 cases in Illinois are rising again. In addition to scheduling your vaccine appointment, please remain vigilant, wear a mask, continue physical distancing and wash your hands frequently.

The Moderna COVID-19 vaccine will be administered at the clinic. This vaccine is recommended for people age 18 years and older and requires two doses given one month apart. You should review the fact sheet about the Emergency Use Authorization (EUA) of the Moderna COVID-19 vaccine, the Illinois Department of Public Health’s COVID-19 Frequently Asked Questions and the COVID-19 Vaccine FAQ on the NIU Protecting the Pack website.

Please do not forward this email. The first-dose Moderna clinic Tuesday April 20, from 9 a.m. to 2 p.m., is a “Closed POD” for NIU employees and their immediate family members only. You will schedule your second dose appointment for May 18 while you are at your first-dose appointment.

To schedule an appointment:

  1. On the calendar, choose a time slot between 9 a.m. and 2 p.m. on April 20. Open time slots are in green. Click on your preferred appointment date and time. Please note the time slots after 2 p.m. are for seniors 65 years of age and up and not for NIU.
  2. Click on “Add to cart.”
  3. Click on “Continue.”
  4. On the “Checkout” screen, complete the required contact information and select “Continue.”
  5. Review your information to ensure it is correct and click on “Submit registration.”
  6. You will receive an email containing your appointment date, time and QR code. Take a screenshot of the QR code and make sure to bring it with to your appointment along with a photo ID and a printed and completed consent form.
