Work Order Request Form

Fill out the form below to submit non-emergency Work Order Requests for Physical Plant craft services and material. You may also submit requests/questions by:

(For remodeling requests, you must fill out a "Request for Capital Improvement" form. Call 753-1768 to obtain the form.)

DO NOT USE THIS FORM FOR EMERGENCIES. For Emergency Services during normal working hours, call the Work Order Desk at (815) 753-1741. If you do not get an answer from the Work Order Desk, call the Heating Plant at (815) 753-1151. If you need emergency services after normal working hours, call the Heating Plant at (815) 753-1151.

STUDENTS - All requests for service relating to residence halls must be communicated through the Residence Hall's "Fix It" telephone line at (815) 753-4948.
