2. Complete Clinic Application Forms
   Please Complete Application Form and Chart: 
   • Fall 2013 Forms
     • Fall 2013 Clinic Application Form
     • Fall 2013 Student Availability Chart
   • Summer 2013 Forms – Due May 2, 2013 
     • Summer 2013 Clinic Application Form
     • Summer 2013 Student Availability Chart
3. Submit Application
   Please Hand-In Completed 711 License and Clinic Application Forms to:
