Select the Account Type for the person who needs the service. This may or may not be the person filling out the form. Once you choose an account type, the available services will appear below. |
| Look Up | When an AccountID is requested, a link is provided to password.niu.edu where you can look up an AccountID by last name. Copy and paste the AccountID to the form. |

Click on the information icon for a definition of the requested information. |
| Rates | If there is a charge for a service, a link to the ITS rates page is provided. A billing account is required whenever there is a charge. |

You can type in a date or click the Calendar button and select a date. Enable pop-ups to view the Calendar. |

The confirmation page lets you review your request. You can go back to make changes. Your request has not been submitted until you press the Submit button at the bottom of this page. |
You can view the status of your request by logging in at MyITS Forms.
You can view Current requests (submitted in the last 30 days)
or All requests.
