ITSForms Guide

status bar The Status bar shows your progress toward submission.
Need Help? The Need Help? link at the top of each page takes you to Helpdesk Central where you'll find contact information for the ITS Helpdesk.
drop down menu for account type Select the Account Type for the person who needs the service. This may or may not be the person filling out the form. Once you choose an account type, the available services will appear below.
Look Up When an AccountID is requested, a link is provided to password.niu.edu where you can look up an AccountID by last name.
Copy and paste the AccountID to the form.
information button Click on the information icon for a definition of the requested information.
Rates If there is a charge for a service, a link to the ITS rates page is provided. A billing account is required whenever there is a charge.
calendar button You can type in a date or click the Calendar button and select a date. Enable pop-ups to view the Calendar.
confirmation page showing submit button The confirmation page lets you review your request. You can go back to make changes.
Your request has not been submitted until you press the Submit button at the bottom of this page.


MyITS Forms

You can view the status of your request by logging in at MyITS Forms.

use the dropdown in the upper right corner to view current requests submitted in the last 30 days or all requests 
You can view Current requests (submitted in the last 30 days)
or All requests.

Notification of completed requests is sent to the requestor's @niu.edu mailbox.
