Immunization Form
Illinois College Student Immunization Act (110-ILCS 2) and University policy states: All students born on or after January 1, 1957, enrolling in class at the NIU DeKalb campus are required to provide written evidence of immune status with respect to certain communicable diseases or evidence of exemption from this requirement.
Health Insurance
Each semester, all international students will be automatically billed for NIU health insurance. Students in F or J status are required to have NIU student health insurance unless approved otherwise in advance.
