Registration

Print and complete the flyer/registration form and mail with your payment to: Office of External Programs, College of Visual and Performing Arts, Northern Illinois University , DeKalb , IL 60115. Your form must be accompanied by your payment. You may pay by check (payable to Northern Illinois University) or credit card. Those who pay by credit card may fax their forms to (815) 753-8372. Fees

Early Bird Fees (must be postmarked by March 1)

• $45 per person
• $35 per person for groups of five or more who register at the same time (each person must complete a registration form)
• $60 per 2.5' x 8' display table (clarinet vendors)

Registration Fees (after March 1)

• $55 per person
• $45 per person for groups of five or more who register at the same time (each person must complete a registration form)
• $70 per 2.5' x 8' display table (clarinet vendors)

Note to people registering as a group: We need a completed registration form for each participant. Photocopy the form as needed for each person.

Note to quartets registering for the Cornucopia Clarinet Quartet Competition: Each individual is expected to register for the day and list the name of the quartet or the other members on the form. An additional $5 is added to the registration fee for each person registering as a member of a competing quartet.

Optional extras

• Box lunch - Box lunches with a choice of turkey or cheese sandwich, piece of fruit, cookie, chips, and bottle of water will be available for $8. Indicate your sandwich preference and the number of lunches you want to order on the flyer/registration form. We will set up tables in Room 155 so the group can eat lunch together. A list of restaurants will be available at the registration table. (Dinner is on your own.)
• T-shirt - Clarinet Cornucopia 2007 t-shirts are black and feature the new logo shown above. To order, note the quantity and size on the registration form. The cost is $15.
• Quartet competition registration - $5 per person competing in the Clarinet Cornucopia Quartet Competition

Hotel accommodations

A block of rooms has been reserved for the nights of March 23 and 24 for participants at the Holmes Student Center Guest Rooms on the campus of Northern Illinois University. The rooms cost $60 (not including tax) for a single or double and may be reserved by calling (815) 753-1444 by March 16. Mention the name of the event to the desk clerk for this reduced rate.
