Program Registration Form (PDF file)
This form is required to register for all NIU Community School of the Arts lessons, classes, ensembles, and programs.  The form for summer activities is available in April.  The form for fall/spring activities is available in late July.  A registration form is also required for those applying for a need scholarship.

Need Scholarship Application Form (PDF file)
Need scholarships provide financial assistance to students 18 years old and younger who are interested in arts instruction, but who are unable to pay the full registration fee. Need scholarships are awarded at the the beginning of fall and spring semesters. In fall 2011, the receipt deadline for applications is August 29, and in spring 2012, the deadline is January 9.

Talent Scholarship Application Form (PDF file)
Six scholarships worth $250 are awarded annually to talented musicians who demonstrate a high level of skill and musicianship. In fall 2011, the receipt deadline for applications is December 7; auditions are scheduled for January 7.

Sinfonia Concerto Competition Form (PDF file)
This competition is open to instrumentalists 18 years old and younger who live within a 75-mile radius of DeKalb. The prize is $200 and the opportunity to perform the concerto with the orchestra. In fall 2011, the receipt deadline for applications is October 24; auditions are scheduled for November 7.

CSA Children’s Choir Scholarship Application Form (PDF file)
Scholarships of $50 - $100 per year are awarded in fall only to choir members based on vocal quality, a student essay, and a teacher recommendation. The deadline is the date of the fall placement auditions, August 31 and September 1.
