1. How do I fill out a Check Request?

The check request is an Excel form.  You will fill in the proper information in the Excel form that corresponds to the information from the purchase or reimbursable item.  For specific information on the check request, please contact the Accounting office.

2. How do I fill out a Travel Voucher?

The travel voucher is a Excel form.  You will fill in the proper information in the Excel form that corresponds to the traveler's information.  Each step of the traveler's journey, needs to be completed on an individual line.  Travelers are allowed one safe arrival phone call during their travel experience.  The meal and hotel information is located under the travel rates tab on the bottom of the form.  For specific information on the travel voucher, please contact the Accounting Office.

3. How do I fill out a Journal Voucher?

The journal voucher is an Excel form.  You will fill in the proper information in the Excel form that corresponds to the information for the journal. There are many different kinds of journals available; included are the journal entry - short and long, interagency form, interdepartmental sales journal, payroll transfer journal form, and the credit card journal entry form.  For specific information on any of the journal forms, please contact the Accounting office.

4. How do I fill out an Purchase Requisition?

The purchase requisition is an Excel form.  You will fill in the proper information in the Excel form that corresponds to the information to be purchased.  If you received a quote from the vendor on products that you are purchasing, please include the quote with the purchase requisition.  If you are making a change order, the authorized signer of the cost center can email the information to the Account Office (a GroupWise resource) with the amount to increase or decrease the purchase order, then they will forward the information on to Procurement Services.  For specific information on the purchase requisition, please contact Procurement Services.

5. How do I enable macros on the FMS forms?

For Excel 2003, open Excel.  Then go to Tools > Macros > Security.  Select Medium.  Re-open the form.  This needs to completed only once. 

For Excel 2007, open Excel.  Then go the Home button > Excel Options > Trust Center > Trust Center Settings > Macro Setting.  Select the radio button that states "Enable all marcos".  Even though Excel mentions it is not recommended, all macros from FMS Forms are safe.

Enabling the macros allows for the forms to calculate and compute the information on the form correctly.  For more information on macros, please contact your system administrator for your computer.

All FMS training is located on the ERP Training Calendar.  The FMS training available to campus is offered on a quarterly basis.
