    • General instructions and help with the form

      The use of live vertebrate animals in teaching, research and testing at Northern Illinois University requires review and approval by the NIU Institutional Animal Care and Use Committee (IACUC) as mandated by Public Health Service Policy and USDA regulations. This Application for Approval form provides the basis for IACUC review. In addition, investigators and participants in approved protocols are required to complete annual training relating to the use of live vertebrate animals. 

      What information do I need to include?

      If more than one species is included in your protocol, enter requested information for each species on separate lines. Similarly, enter requested information on each additional participant on separate lines. If there is a grant proposal associated with your protocol, provide the Office of Sponsored Projects (OSP) number and attach the ‘Vertebrate Animals’ portion of your proposal. Sections concerning Euthanasia, Endpoints and Occupational Health & Safety must be completed for all protocols.

      You are expected to provide appropriate literature citations throughout your protocol.

      Information on CITES (Convention on International Trade in Endangered Species of Wild Fauna and Flora) species can be found at the CITES Website (click on species database). Information on euthanasia methods approved by the AVMA Panel on Euthanasia can be found here.

      Applications must be complete in order to be considered by the IACUC. Protocols needing minor modifications or clarifications may be approved on a provisional basis. Final approval will not be granted until requested modifications or clarifications are provided. No procedures may begin before final approval.

      Instructions for using the OnBase IACUC Form

      Log in: Log in using your NIU user ID and password to access the IACUC form and instructions. Click the “Application for Approval” tab to begin filling out the form. 

      1. Save Your Document: We advise you to save your document immediately upon access. This generates a protocol-specific number and establishes your IACUC application in the system. This first time you save your document, a blank screen will appear and an alert will verify that you have saved but not submitted your protocol. You will then receive an email containing a link that will allow you to resume access to continuing edits (this process may take several minutes). Each additional time you save your document, you will remain within the editable document.
      2. Edit Your Document: This form can be completed over several days. Simply click the “Save” button each time you want to close the form. When you are ready for final submission, check the “Ready for Final Submission” checkbox and then click “Save”. An alert will indicate that your protocol has been routed for IACUC review.
      3. Text Boxes will expand to accommodate as much text as you wish to provide.
      4. Cut and Paste from Other Documents: Cutting and pasting from other documents into text boxes is possible using keyboard controls (e.g., Ctrl-V, Open Apple V). Cutting and pasting using the mouse (i.e. right clicking) are not supported by this system. NOTE: If cutting and pasting text, make sure to tab out of the field to ensure the text is saved.
      5. Submit Your Application: Submit your application by checking the box indicating, “Ready for Final Submission”, located at the end of the document, then clicking on the “Save and Submit”.
      6. Routing of Applications: Once completed and submitted, your application will be automatically routed for appropriate signatures (Faculty Mentors for students, Department Chairs) and submitted to the Office of Research Compliance where it will be processed for IACUC Review.

This Application for Approval form provides the basis for IBC review. In addition, investigators and participants in approved protocols are required to complete annual training relating to the use of biohazardous material.
