Use this form to report general incidents of misconduct. Incidents include, but are not limited to:

  • Alcohol use
  • Drug use
  • Housing policy violations
  • Physical assault
  • Organizational violations
  • Hazing incidents
  • Disruptive behavior

This form is not used to report sexual or academic misconduct.

Use this form to report incidents of sexual misconduct and behavior that violate Title IX.

Housing and Residence Life staff use this form to report individuals involved in medical or traumatic situations. It should be used anytime someone seeks or needs medical attention, regardless of transport to the hospital

Routine Maintenance Issue

Housing and Residence Life staff use this form to report routine maintenance issues, in the residence halls. This form should not be used to report damage, destruction, or vandalism to property. Issues of damage, destruction or vandalism should be reported through , with the nature of the report set to Damage, Destruction, Vandalism.
