Students who have been dismissed from the College of Health and Human Sciences may appeal that decision by submitting a formal letter of appeal to Dean Shirley Richmond.

Appeal letters should include details relative to the following factors:

1) information about circumstances that lead to academic difficulty
2) why things would be different if allowed to continue
3) any other relevant factors

Appeal letters may be submitted by fax at 815-753-6169 or by mail to:

Letters should include Z-ID, telephone number, return address, and student signature. Appeals should not be sent by email.

Students will be notified, in writing, of the Dean's decision. Please note, campus offices are closed beginning Thursday, December 24 and will re-open at 8:00 a.m. on Monday, January 4, 2010.
