Guidelines for Documentation

Necessary Documentation

To request and initiate services, students must submit to CAAR the necessary and appropriate official documentation verifying the disability. The type of documentation may vary dependent upon the type of disability. Documentation should be current and include the following :

• Attention Deficit Disorder
• Learning Disability
• Blind/Visually Impaired - If a student has had a low-vision evaluation, a copy of the evaluation should be included with the documentation. 
• Deaf or Hard of Hearing
• Physical/General Disability
• Psychiatric Disability
