  • Print and complete the catering forms, which are pdf's. Mail or fax to 815-753-8787.
  • Completed catering forms must be received 7 working days before the date of service.
  • You must receive a signed fax confirmation of your order to guarantee service.

The total on the form is an estimated total. The actual total will be billed after the event due to possible additional needs or requests.
